Patient Assistance Program
Dialysis and chronic kidney disease do not stop the rest of life from happening. Our Patient Assistance Program helps Mississippi kidney patients cover the everyday costs that get in the way of treatment. Applications are submitted by the social worker at your clinic, requests are reviewed on a case by case basis, and payments are issued directly to vendors.
Who Qualifies
To be eligible for assistance, an applicant must:
- Be a diagnosed chronic kidney disease (CKD) patient, including patients receiving dialysis
- Reside in the state of Mississippi
- Be under the care of a clinic or provider with a social worker who can verify and submit the request
- Have a documented need tied to one of the covered categories below
What We Help With
Transportation
Rides to and from treatment, fuel, and related travel costs that keep patients on schedule.
Utilities
Help with a past due or upcoming utility bill so home treatment and daily life stay uninterrupted.
Groceries
Support for food and renal friendly nutrition needs.
Co-Pays
Assistance with treatment related co-pays and prescription costs.
Medical Supplies
Supplies connected to dialysis and kidney care that are not covered elsewhere.

What we do not cover: The Manors Foundation does not pay rent or provide housing assistance at this time. If housing is your primary need, please speak with your social worker about local and state programs that may be able to help.
How Much, and How Often
Each approved patient may receive up to $200 in assistance per calendar year. That amount can be used all at once or split across more than one request until the annual limit is reached, so you are never forced to spend it on the first need that comes up.
The annual limit resets every January 1. Assistance is limited and based on available funding, and all payments are made directly to the vendor rather than to the patient.
Where We Serve
The Patient Assistance Program is open to kidney patients throughout the state of Mississippi. We currently work with patients in multiple dialysis clinics across the state, and we are actively expanding to reach every Mississippi patient who needs us. If your clinic is not yet familiar with the foundation, have your social worker reach out and we will get them set up.
What You Need to Apply
There is no separate form to download, print, or scan. Verification is completed inside the application itself. To submit a complete request, your social worker will need:
- Your name, date of birth, address, and contact information
- The type of assistance needed and a short description of the need
- An invoice, bill, or statement from the vendor to be paid
- Their own name, title, facility, and signature, entered directly in the application

Requests are reviewed within 48 hours of receipt.
Vendor Payment Issued
Approved funds are paid directly to the vendor.
How Long a Decision Takes
! Our Timeline
Complete requests are reviewed within 48 hours of receipt, and a decision is rendered shortly thereafter. You and your social worker will both be notified of the outcome using the contact information on the application.
Incomplete applications cannot be reviewed, so the vendor invoice needs to be attached before the application is submitted.
! You Do Not Apply Alone
You do not need a computer, an email address, or internet access to get help. Start by speaking with the social worker at your clinic. They submit the application on your behalf, and they are your point of contact throughout the process.

Assistance is limited and based on available funding. Meeting the eligibility requirements does not guarantee an award. Last minute or urgent needs may be considered on a case by case basis depending on the availability of resources.
